Regulations for visit services provided by electronic means<\/b><\/p>

\u00a7 1 General provisions<\/b><\/h2>

1.1 These Regulations are Regulations within the meaning of Article 8 of the Act of 18 July 2002 on the provision of services by electronic means (Journal of Laws No. 144, item 1204); they are intended for Service Recipients using the Website available at: www.optimumpsyche.com<\/a> and specify the manner and principles of purchasing visits and using psychotherapeutic services provided via the Internet or telephone.<\/p>

1.2 The owner of the Service is Joanna Mania running a business Joanna Mania based in Gdynia, address: ul. Marii Curie-Sk\u0142odowskiej 10a\/2, NIP: 5842433423<\/p>

\u00a7 2 Definitions of terms<\/b><\/h2>

2.1 Terms of Service are these rules and regulations that specify the manner and principles of purchasing appointments, as well as changing the appointment, withdrawal from the contract, additional fees and complaints.<\/p>

2.2 The Service is the website available at www.optimumpsyche.com<\/a>, through which it is possible to purchase appointments.<\/p>

2.3 The owner of the website is Joanna Mania running a business Joanna Mania based in Gdynia, address: ul. Marii Curie-Sk\u0142odowskiej 10a\/2, NIP: 5842433423<\/p>

2.4 The Service Provider is a psychotherapist who provides services by conducting visits.<\/p>

2.5. the Customer is a person who purchases a visit through the Service.<\/p>

2.6 A Client is a person using a service purchased through the Service.<\/p>

2.7 A visit is a psychotherapy service provided by a psychotherapist hereinafter referred to as the Service Provider via the Internet or telephone.<\/p>

2.8. Application form is an electronic form available on the Site, the completion of which is necessary to purchase a visit.<\/p>

2.9. The conclusion of a contract is the purchase of a visit requiring the completion of the application form, acceptance of the terms and conditions of services and payment. The conclusion of the contract is confirmed by the email that the Service Recipient receives after payment for the service. The purchase of a visit constitutes the conclusion of a contract between the Service Provider and the Client.<\/p>

\u00a7 3 Types, scope and duration of psychotherapeutic services.<\/b><\/h2>

3.1 The Service offers the following psychotherapeutic services provided by the Service Provider.<\/p>

3.2 The objectives and method (e.g. frequency of visits, duration of visits, working methods, type of communicator) of providing the service are determined individually between the Service Provider and the Service Recipient and\/or the Client based on the information provided by the Client, the state of his\/her psychophysical health and his\/her real possibilities and needs.<\/p>

3.3 The description and general rules for the use of individual psychotherapy services are located in the sub-pages of www.optimumpsyche.com<\/a> \u201cAbout me\u201d, \u201cOffer\u201d, \u201cTherapeutic process\u201d and \u201cFAQ\u201d.<\/p>

\u00a7 4 Purchase of visits<\/b><\/h2>

4.1 Purchase of visits is made electronically using the application form located on the Site. It requires filling in the application form, accepting the Terms of Service and making payment.\u00a0<\/p>

4.2 Completion of the application form requires filling in the required data and selecting a visit date in accordance with the Polish time zone.<\/p>

4.3 In case of lack of due payment (concerning e.g. use of coupons to which the Client has not been entitled or failure to adjust the payment to the number of participants of the visit) the service will not be provided.<\/p>

4.5 In the aforementioned case, the Customer shall not be entitled to a refund of the costs incurred for the purchase of the service.<\/p>

4.6 The owner of the service shall not be liable for any hindrance to the purchase of a visit caused by a possible defect in the payment system.<\/p>

\u00a7 5 Changing appointments<\/b><\/h2>

5.1 It is possible to change the appointment through the Client Panel – from the moment of receiving confirmation of the visit purchase, but no later than 24 hours before the start of the visit.<\/p>

5.2 In the aforementioned case, the appointment chosen by the Client must be accepted by the Service Provider. The Client will receive confirmation of the appointment change by email.<\/p>

5.3 There is no fee for rescheduling an appointment.<\/p>

\u00a7 6 Withdrawal from the contract<\/b><\/h2>

6.1 The purchase of an appointment involves an obligation to pay. The Customer has the right to withdraw from the contract within 14 days from the date of conclusion of the contract.<\/p>

6.2 The Customer has the right to withdraw from the contract by cancelling the visit in the Customer Panel available on the Site.<\/p>

\u00a7 7 Fees and additional charges<\/b><\/h2>

7.1 Services provided by the Service Provider through the Site are subject to fees in accordance with the applicable price list available in the \u201cPrice List\u201d tab.<\/p>

7.2 Electronic invoices shall be sent to the email address provided in the application form.<\/p>

7.3 In case of cancellation of an appointment within less than 24 hours from the start of the appointment, the Customer shall pay an additional fee to the Service Provider in the amount of the cost of the service.<\/p>

7.4 In case of absence of the Client during the purchased visit, the Client shall be obliged to pay to the Service Provider an additional fee in the amount of the cost of the service.<\/p>

7.5 The Service Provider reserves the right to deduct the aforementioned additional fee in the indicated amount from the amount paid by the Client as payment for the purchase of a visit.<\/p>

\u00a7 8 Complaints<\/b><\/h2>

8.1 The Client has the right to lodge a complaint if the services are inconsistent with these Terms and Conditions or the provisions of generally applicable Polish law within 7 days from the end of the visit.<\/p>
